Provider had wires hanging from the counter top, provider had phone charger plugged in and accessible to children. Provider had plastic bags on top of the kitchen counters that are accessible to children. Provider had large items in the closet, the closet door can not close. Provider has a cabinet full of toys that was shaky. Bathroom floor had flaking paint. Water temperature was 134 hotter than 120.
